To use your STU,
- You need to exhaust your deductible.
- Make a claim as a reimbursement.
a) If you have left the organization you were a part of while purchasing STU- reach out to Care Health insurer directly to raise a claim
b) If you are still a part of the organization- Contact Plum
Share all documents on the checklist with us over mail at care@plumhq.com, and the concerned team will take it forward.
Document checklist:
1. Claim form- Care Health
2. Proof of bank account (cancelled cheque)
3. Discharge summary
4. Final hospital bills
5. All investigation reports
6. All pharmacy bills
7. ID proof of the patient
8. Settlement letter of claims/Proof that deductible has been exhausted.
