1. Discharge Summary (Main Hospitalization)

2. All Bills and Payment Receipts

3. All Investigation Reports

4. Doctor Consultation Letters/Prescriptions

5. Consultation Bills

6. Pharmacy Bills (if any)

7. Proof of Bank Account of the Employee ( Cancelled Cheque, Bank Statement) 

8.. ID Proof of the Employee and the Patient
