What documents are required to file a claim? (OPD)

Here is the list of documents required to file a reimbursement claim for Outpatient Department ( OPD) treatments: 

1. All Bills and Payment Receipts 

2. All Investigation Reports

3. Doctor Consultation Letters/Prescriptions

4. Consultation Bills

5. Pharmacy Bills (if any)

6. Proof of Bank Account of the Employee ( Cancelled Cheque, Bank Statement) 

7. ID Proof of the Employee and the Patient
